Registration of new patients
To receive all services from Moorside Surgery including home visits and other out-of-hours services we will need to see proof that you live within the practice boundary. This is normally in the form of a utility bill, bank statement or similar document dated within the last 3 months and showing your current address
We have produced a 'New Patient Registration Pack' to make the registration process easier. These are available from reception or to download and print below. It includes some forms which you will need to complete and return with your proof of address and ID. You will then be asked to make an appointment with the practice healthcare assistant for your new patient health check.
Find your NHS number
You do not need to know your NHS number to use NHS services, but it can be useful to have it
Non UK residents can be seen as private patients - please ask at reception for a price list.

Please ensure you to into account we run busy urgent care clinics daily from 10.00am until 12.00pm so it may be best to avoid registering during these times.
